•	I agree that I am over the age of 18 and acknowledge that I choose to receive services from The Nailroom by SK and have voluntarily filled out this consent form  •	I agree and acknowledge that I am receiving services from a volunteer nail professional and not a medical practitioner.  •	I agree_ I grant permission for The Nailroom by SK to use and/or apply any necessary products as part of the service I am receiving.  •	I agree to conduct myself appropriately while receiving this service.  •	I agree to the best of my knowledge, I have no health condition or allergies that would prevent me from receiving nail services. •	I understand that nail products contain reactive chemicals which may result in allergic reactions in some individuals and hereby assume all personal risk.  •	I agree that The Nailroom by SK reserves the right to deny services if the client is present with any nail or health conditions that may be contraindicated for receiving nail services.   •	I understand that it is out of the scope of the nail technician to diagnose health conditions or to give any medical advice.   •	I authorize The Nailroom by SK to take photos before, during, and after my nail service and authorize the use of these photos solely for the purpose of The Nailroom by SK internet and portfolio publication, social media, or promotions and advertising.   •	By submitting below, I am aware of and acknowledge any unexpected results or issues that may arise with the nail services I receive. I am fully aware of any risks involved in the services I receive, including any adverse reactions or allergies. I agree to voluntarily release, forever discharge, and agree to indemnify and hold harmless The Nailroom by SK from any and all losses, claims, injuries, or damages, sustained or incurred by me arising out of or related to the services and/or treatments rendered by The Nailroom by SK or results thereof, including but not limited to any claims arising from the treatment of my nails, the application of any nail products to my nails, including, but not limited to, claims based on any allergic reaction, damage or injury to my nails or skin, or any other claim and shall be responsible for all attorney's fees and costs incurred as a result of such claim.
